April 2, 2026

To all patients and families, colleagues and friends,

Collin County Pulmonary Associates, the office of Dr Tim Chappell, will permanently close on June 30, 2026. There are several key factors in this decision:

  • First, our office lease expires this summer.
  • More significantly, Janet, our office manager for 30+years has decided to retire.
  • Simultaneously, Brenda, our long time Xray and PFT technician is leaving to be a full time Radiology Technician student.

While no one is irreplaceable, these two come pretty close!! The confluence of all three factors would mean our office practice would essentially be starting over again. We were going to rebuild until a personal and professional opportunity unexpectedly arose that I just am not able to pass up at this point in my career.

In August, I will be relocating to Lima, Peru for immersive medical Spanish language training in preparation for serving as a medical missionary in Guatemala. My initial assignment in Guatemala will be for the rest of this year. Whether or not it extends into 2027 depends on if it is as personally and professionally fulfilling as l expect it to be. This decision is not quite as abrupt as it may seem.

In 2016, I was planning to take a six month sabbatical to do this mission with my wife Peggy (who was a nurse). Her health changed and this opportunity, at least for her, was lost when she passed away last year. When I was contacted to see if we might still be interested in a mission trip, I knew it was now or never, and I just had to say yes for both of us to fulfill our dream – at least for me.

Whenever I do come home, my plan is to start over and return to clinical practice as an employed physician. Those details will then be available here our website.

Janet has agreed to work from home for the rest of this year, but only just to help with medical record requests. Janet cannot help with – and Dr Chappell will be completely unavailable for – anything and everything such as office visits, hospital care, medical questions and phone advice, portal messaging, prescriptions and refills, certifications and renewals of any medical equipment or supplies and prior authorizations, etc.

For continuity in your ongoing care and to help you transition to a new pulmonologist, please see both the pulmonary doctor list and the necessary forms for medical records release on our website below.

I have been blessed to know so many of you professionally and personally. Thanks to each of you individually and all of you collectively on behalf of myself and our office staff for being so friendly, helpful and cooperative with us as we guided and managed your pulmonary care. We have been honored by the opportunity to have been your pulmonary office.

I want to thank everyone of you for being such a big part of a great 45 years as a pulmonary doctor and on behalf of the practice we wish every one of you the best health, happiness and personal fulfillment.

Gratefully yours,

TR Chappell, MD
